Creating and Updating Review Forms

Review Forms are very flexible and can be used in Review Rounds. They're ideal for use cases where you want everyone in a review round to answer the same questions.


For example:

  • Setting up a Self Reflection form that all employees will complete
  • Setting up an upward review with the same set of management and leadership questions

Creating a Review Form

Navigate to Reviews >> Forms on the sidebar. This will take you to a list of your review forms.


Click the Create Form button to create a new one.



Updating the Title and Introduction

Name: A simple name for this form.

Introduction: This gets shown to each feedback provider for this form. It can be used to add additional context if needed.



Building the Form

Navigate to the Form tab at the top right. This will take you to the page where you can add sections and questions to the form.


Sections are an easy way to group questions.


Review Questions can be added to any section. They are open-ended or custom rating questions.


You can reorder by the sections and their questions.
